Registration

• Download the 2023 Sports-Registration Packet
• Read, complete, and sign the form, along with the physical-exam form or obtain one from your doctor.
• Return the completed Sports Registration Form, including the physical exam paperwork, as a hard copy to Liberty Common High School athletic department or front office. You only need to submit the physical and form once per year, but you must do this every year.

Athletes must have everything completed by the deadline below in order to be eligible for practice on the first day of practice.

• Men’s Golf – July 31st
• HS Fall Sports – August 7th
• JH Fall Sports – August 21st
