Registration and Forms

• Parents will submit the CHSAA Preparticipation Physical Evaluation Form through PowerSchool. Only the official CHSAA physical form will be accepted, no other physical will be allowed. This physical will be identified via the CHSAA logo in the top left corner. 

• The upload platform can be found on the left-hand side of the "parent view" in PowerSchool under "forms."

• Only CHSAA physicals are accepted. No paper copies will be accepted.

• Once submitted, the online form remains valid for the entire school year. 

• The physical is valid for 365 days from the exam date. A valid physical must be on file to participate in the season.

• A student doing a second or third sport must re-submit their powerschool form with the new sport selected. 

• Payment will be done on RevTrak. 

• Select the season you wish to register for, then the sport. 

• Payment must be completed prior to Step 3.

• Students who have played two sports this scholastic year are eligible to have the fee waived for their third and fourth sport. Contact Mr. Russel to let him know your intent to participate.  

• After submission of forms and confirmation of payment, students must visit the athletics office at Liberty Common High School to pick up a signed blue card from the athletics department, who will verify steps 1 and 2 have been completed.

• A staff member will be available at the elementary and junior-high campuses to sign blue cards.

• A blue card is required for participation. Without it, students will not be allowed to practice or compete.
