Registration for all athletics is completed through R evTrak . Please contact athletics@libertycommon.org if you have any questions.

Sports-Registration Packet

Download the Sports-Registration Packet
Read, complete, and sign the form, along with the CHSAA Preparticipation Physical Evaluation Form.
Return the completed Sports Registration Form, including the physical exam paperwork, as a hard copy to Liberty Common High School athletic department or front office. You only need to submit the physical and form once per year, but you must do this every year.

Deadlines

Athletes must have everything completed by the deadline below in order to be eligible for practice on the first day of practice.
